Step 3. A Customized Plan of Care is developed based on your assessed needs and requests.
Step 4. Coordinated and Supervised Services is one of the advantages of in-home services which ensures that a care recipient’s needs are being met. As care needs increase or decrease, hours can be adjusted accordingly.
